These forms may be completed online and e-mailed to our office at drjamesbaran@gmail.com or printed and completed at home. You can then either fax the forms to 412-856-5471, or bring them with you to your appointment.
Your first visit to Colo-Rectal Associates establishes an essential foundation for our relationship with you. During the first visit, we make sure to obtain important background information, such as your medical history, and give you time to get to know Dr. Baran. Dr. James Baran is a private provider at Colo-Rectal Associates, most insurances are accepted, however, it is the patients responsibility for pre-verification of eligibility and it is encouraged for all patients to contact their insurance carrier and verify if Dr. Baran is an in-network provider prior to scheduling appointments. Please check with your insurance if Dr. Baran is an in-network provider with your plan at the Monroeville or Mars office, our location addresses can be found under the 'Contact' page.
